If you’re interested in receiving treatment such as IVF, ICSI, the use of donor sperm or eggs or any of the other services we provide to help you have a family, please click the ‘Self-Referral Questionnaire’ link below. You will be directed to a simple form to fill out, giving us some basic background about you. We’ll then get in touch with you directly.

If you’re interested in joining our egg-sharing programme – where you receive a cycle of IVF at a subsidised rate for donating some of your eggs to other women – then please click on ‘Egg-sharing programme’ below where you can provide us with some details about yourself. We will then assess your information to see if you’d be suitable to become an egg-sharer.
